Brings to mind a story that happened here in the Phoenix area about 8 or 9 years ago.
An Asplundh (tree maintenance company) crew was trimming and maintaining some trees along a street when a guy walks up to the crew, doesn't say a word, takes his wallet out of his pants, takes off his sunglasses and dives head-first into the chipper!

PORTLAND, Ore. (KOIN) — A man died Friday when he fell into a meat blender at a Clackamas plant, Oregon OSHA officials confirmed.
Hugo Avalos Chanon, a 41-year-old from Portland, worked for DCS Sanitation Management, according to the Clackamas County Sheriff’s Office. He was cleaning the facility at Interstate Meat Distributors, 9550 SE Last Road, with several other people when he fell.
A co-worker hit the emergency button to stop the meat blender. But it was too late.
A meat blender is used to blend the meat and the fat.
This isn’t the first violation at the plant.
OSHA inspection documents found a problem on three machines “in the case-ready area and grinding room.” The documents show that the machines were not “locked out” during the tear-down process for cleaning. According to OSHA’s February report, the failure to lock the machines exposed workers “to the potential unexpected start up of the machine — resulting in serious injuries to the finger(s)/hand(s).”
The company was expected to fix that problem by March 11, 2013.
Oregon OSHA reported it’s looking into this newest incident, but said the investigation could last up to six months. Interstate Meat Distributors remains in operation
